Enabling a pathway for each person into the community; with the relevant support around them

Pathway: Step

2 bedroom properties (1 room would be for staff) are available in the Northwest area depending on the needs of the individual and other stakeholders. All properties provide kitchen space, living room, dining area, staff room and private garden or yard.

A bespoke care package will be tailored around the individual. Option of 1:1 support as a minimum (or 2:1 if required at additional cost), sleep-in staff and or waking night member staff will be present. A dedicated Senior Support Worker will be available to support the individual throughout their journey. 24 hour on-call senior management support and guidance. Oversight and guidance from Registered Manager.

All frontline staff will complete; the Care Certificate, and as a minimum standard. They will acquire the NVQ Level 2 in Health and Social Care, further opportunity to achieve L3, L4 and L5. Specialist training events will be provided by Dr Rogers specific to each person that is supported. Further specialist training from a Counsellor in relation to therapeutic engagement will be offered.

Each person that is supported will receive a robust care package with careful planning to clearly determine how they can be supported in the community in their own home. There will be no maximum stay in the service, as each person progresses as their own pace, however, there will always be an emphasis on progression.

The measure of progression is detailed within a service specific tool that allows each person entering the service to set their own goals, and easily identify how they will achieve them.

The use of technology will be implanted where possible in order to reduce operational costs, this allows for a reduction on cost of service to the commissioning body. In addition, the use of technology will be used to improve efficiency.

Each individual will sign their own tenancy agreement to enable them to step forwards towards independence. The rate is set at a subsidised rate in partnership with Social Care Landlord.

Every person entering our service is given opportunity to shape the service they receive by various engagement activities such as: 1 -1 meeting with senior management, open sessions for response, and also regular group forums led by the people that we support.

Pathway: Transition

2/3 bedroom properties (1 room would be for staff) are available in the Northwest area depending on the needs of the individual and other stakeholders. All properties provide kitchen space, living room, dining area, staff room and private garden.

24 hour staffing support is present within the setting to provide ongoing and consistent support to each individual. Consistent 1:1 support as a minimum (or 2:1 if required at additional cost), sleep-in staff and also a waking night member of staff to be present in the setting during the night. A dedicated Senior Support Worker who supports each individual prior to entering the service and through to preparing to the Transition stage. 24 hour on-call senior management support and guidance. Rigorous Clinical Risk review and governance, including regular clinical oversight from a Nurse Assessor. Enhanced costs may be applied for immediate access to specialist services for example Speech and Language Therapist and Physiotherapy.
